Maybe you could consider the Maudsley Addiction Profile (MAP). The MAP is a multi-dimensional instrument for people with drug and/or alcohol problems, which can be easily administered at intake, during, and after an index treatment episode. It contains 60 self-report items measuring problems from four domains: substance use, health risk behavior, physical and psychological health, and personal/social functioning. The average completion time of the MAP is 12 minutes. According to the existing literature, the MAP questionnaire is acceptable to most subjects and performed well with both researcher and clinician interviewers. Internal reliability and feasible concurrent validity assessments of the scales and items are also highly satisfactory.
