The potential choice depends on the program's main goals. Based on the purpose and target groups of your intervention, you may consider the following ones:
1) Patients' knowledge and self-efficacy have increased.
2) Patients' health-related behaviors have changed.
3) Rates of provider adherence to guidelines have increased.
4) Disease control has improved.
5) Patient health outcomes have improved.
6) Patient satisfaction has improved.
7) Utilization has declined, and patient productivity has improved.
8) Health care costs have declined.
