Social workers may be involved in an interdisciplinary team in both outpatient and inpatient programs designed to help persons with dementia. These include outpatient programs such as memory clinics that provide multidisciplinary assessment and care plans (often involve a team of geriatricians, nurses, and social workers) to help caregivers maintain persons with dementia in community settings.
