Questions setting for my targets

I am working at a NGO targeting the elderly. Sometimes we conduct survey to measure our effectiveness of our campaigns but the elderly seems annoying or hesitated when we ask them the scale of each question. It seems that they don't know how to choose the 'degree' of the description. Shall I just switch my 5-point scale questionnaire to 2-point scale or qualitative research?

It is a great suggestion Dr MEL, I also think having assistance to fill out the survey is important for the elderly. Also keeping the survey short and simple.

Please note that collapsing the number of the scale points to two is a viable way.

An alternative is to ask their family members to report their observations and experiences.
