Here are five points to consider when evaluating a program aiming at assisting inmates in their preparation for discharge by adopting early intervention and multifarious service models:

Collaboration and coordination: Assess the level of collaboration and coordination between various stakeholders involved in the program, such as correctional staff, social workers, mental health professionals, educators, and community organizations. This includes evaluating the effectiveness of communication channels, joint planning, and the sharing of resources. Strong collaboration ensures a comprehensive and integrated approach to supporting inmates' preparation for discharge.

Empowerment and self-efficacy: Evaluate the program's impact on participants' sense of empowerment and self-efficacy. This involves assessing changes in participants' confidence, decision-making abilities, problem-solving skills, and their belief in their capacity to lead a successful life post-release. A successful program should foster a sense of empowerment and equip participants with the skills and mindset necessary to overcome challenges.

Family and social support: Assess the program's effectiveness in facilitating and strengthening family and social support networks for inmates. This includes evaluating the provision of services that promote positive family relationships, facilitate reconnection with loved ones, and foster community support networks. Strong family and social support systems are crucial for successful reintegration and reducing recidivism.

Individualized case management: Evaluate the program's approach to individualized case management for inmates. This includes assessing the development and implementation of personalized release plans, tailored to each participant's specific needs, strengths, and challenges. Individualized case management ensures that participants receive appropriate services and support based on their unique circumstances.

Program sustainability and replicability: Assess the program's sustainability and potential for replication in other correctional facilities or jurisdictions. This includes evaluating the program's scalability, cost-effectiveness, and the availability of resources to ensure its long-term viability. A sustainable and replicable program can extend its positive impact to a broader population of inmates and contribute to systemic change.

Hi, I realized that it seems the concept “outcome” is different from the term “output”. Taking the grandparent training project as an example, what is the “outcome’ referable?
