If your participants are under 18, make sure you get parent’s consent as well. You should describe what your study is about in the form, let participants know their rights like: they can leave the study at any time, they can skip questions they don’t want to answer etc. and how will their data be used/handled.

Could you perhaps assist me in getting some information about the community service orders (CSO) program verified? According to what I’ve heard, a court may issue a CSO order requiring a person who has reached the age of 14 or older and has been found guilty of a crime carrying a sentence of imprisonment to perform unpaid work for a number of hours up to 240 hours per month while being supervised by a probation officer. Can I refer to those as CSO scheme activities?
