As care coordinators, social workers take greater responsibility for implementing the care plan they have developed with family members and persons with dementia. Social workers help to link persons with dementia and their informal caregivers to suitable community resources and services and help to ensure that these services are received in a timely and consistent fashion by resolving any issues related to the implementation of the ongoing care plan.
