If you do not understand what your client is communicating, ask him/her to repeat what has been said. If you need to do this repeatedly, you might want to acknowledge this by saying that you're a poor lip reader and please excuse this or that you know it is hard for him/her to repeat things so often. Still, you want to be sure you have heard accurately what he/she is saying. If your client sees that you are making a genuine effort, he/she will generally work hard to communicate with you.
Some people are just naturally poor lip readers. No matter how much they practice or how hard they try, they cannot extract meaning from the client's lip movements. If you are one of those people, explain this to your client and suggest trying alternative methods of communication rather than risk creating frustrations on both sides of the encounter.
