The assessment focuses on the impairment, but the home care service provision is based on the judgment and advice of professionals.

Elderly persons who are assessed to be moderately or severely impaired by the Standardized Care Need Assessment Mechanism for Elderly Services and have the genuine need for a well-coordinated package of home care and community support services can suit the “frail cases” in Integrated Home Care Services teams.
