1. Screening during Intake or referral process: Incorporate screening questions related to social and psychosocial needs into the intake or referral process. 2. Electronic medical records: Collaborate with healthcare providers to access electronic medical records (EMRs) of patients. EMRs can provide valuable information about a patient's diagnosis, treatment history, and any documented psychosocial concerns.
