You are cordially invited to join the Community of Practice Session (CoP). Mr. Tseng Ka Chun, Programme Officer-Team Leader, Catholic Diocese of Hong Kong Diocesan Pastoral Centre for Workers-Kowloon, Ms. Cecilia Choi, Pharmacist and Project Manager, Hong Kong Pharmaceutical Care Foundation, and Mr. Ivan Chau, Pharmacist, Hong Kong Pharmaceutical Care Foundation, alumni from this Project, will be invited to share their experience on adopting MEL knowledge on program evaluation and monitoring on 27 August 2024 (Tuesday) from 2:00 pm – 3:15 pm via Zoom.

Cook-Fit Healthy Cooking Workshop for South Asian Families

Description:

In 2023, we conducted a pilot programme titled “Cook-Fit Healthy Cooking Workshop for South Asian Families.” This initiative aimed to understand the causes of obesity within the South Asian community. The programme included three sessions combining knowledge sharing, shopping practice, and cooking. Key challenges included adopting new recipes while preserving traditional cooking methods and concerns about family acceptance of a healthy diet. We evaluated cognitive changes through home visits and social media follow-ups.

Medication Management Service to Stroke Patients – Service and Learnings

Description:

Medication management service aims to enhance personal medication management ability of elderly patients through individual consultation sessions by pharmacists. We will share our MEL project on why we extended the service target group to stroke patients and how we performed quantitative evaluations. Then we will talk about our learnings on the management and utilisation of data.

Introduction of the Jockey Club MEL Institute Project:

A 3-year programme, Jockey Club MEL Institute Project, funded by The Hong Kong Jockey Club Charities Trust, aims to further promote organizational-level MEL (Monitoring, Evaluation and Learning) capacity by providing holistic training to local NGO practitioners. Participants from different social services and professional backgrounds in local NGOs will complete a 6-month programme, which consists of a 7-week training session and a 4-month mentored practicum.
